Hyperglycemic memory refers to the deleterious long-term effects of previous periods of poor glycemic control on diabetic complications, even after glucose levels are subsequently improved or normalized. This phenomenon represents the negative counterpart to the glycemic legacy effect, where tissues retain 'memory' of past hyperglycemic exposure through epigenetic modifications, advanced glycation end-products, and structural changes that continue to promote complications despite improved current glucose control. This concept explains why some patients with historically poor control remain at high risk for complications even after achieving target glucose levels.
