Protamine sulfate is a positively charged protein derived from fish sperm that neutralizes the anticoagulant effects of heparin by forming inactive complexes. It is routinely administered at the conclusion of cardiopulmonary bypass to restore normal coagulation function. Protamine administration requires careful monitoring as it can cause severe allergic reactions, pulmonary hypertension, and cardiovascular collapse in susceptible patients.
