Target lesion failure (TLF) is a composite endpoint specifically designed to assess outcomes related to the treated coronary lesion in interventional cardiology trials. TLF combines three components: cardiac death, target vessel myocardial infarction (not clearly attributable to non-target vessels), and clinically-driven target lesion revascularization. This endpoint provides a lesion-focused assessment of treatment efficacy and safety, distinguishing events directly related to the index lesion from those occurring elsewhere in the coronary circulation. Target lesion failure has become a standard endpoint in percutaneous coronary intervention trials, particularly those evaluating drug-eluting stents and other interventional devices.
