Periodic lateralized epileptiform discharges (PLEDs) are distinctive EEG abnormalities characterized by sharp waves or spike-wave complexes that occur at regular intervals of 1-4 seconds, typically confined to one hemisphere. These patterns often indicate acute or subacute brain injury, such as stroke, encephalitis, or brain tumor. PLEDs are associated with increased seizure risk and may represent a continuum between interictal and ictal activity. The pattern may evolve over time, and continuous EEG monitoring is often required to assess for subclinical seizures.
