Full Definition

Intraoperative cholangiogram (IOC) is a radiographic procedure performed during biliary surgery to visualize the bile duct system using contrast material injected through the cystic artery or choledochotomy. In emergency surgery, IOC helps identify retained stones, ductal injuries, or anatomical variants that could affect surgical outcomes. The procedure involves injecting contrast under fluoroscopic guidance and interpreting real-time images to ensure ductal patency and absence of filling defects. IOC is particularly valuable in emergency cholecystectomy for acute cholangitis or when ductal injury is suspected during difficult dissection.
