CAPTEM is a chemotherapy regimen that combines oral capecitabine and temozolomide, primarily used for treatment of advanced pancreatic neuroendocrine tumors (PanNETs). The regimen was developed based on the rationale that capecitabine depletes O6-methylguanine-DNA methyltransferase (MGMT), thereby enhancing the cytotoxic effects of temozolomide. Clinical studies have shown promising response rates, particularly in pancreatic NETs with lower MGMT expression. The typical dosing involves capecitabine 750 mg/m² twice daily for 14 days and temozolomide 200 mg/m² daily for 5 days, repeated every 28 days.
