Gastric neuroendocrine tumors (gastric NETs) are rare tumors arising from enterochromaffin-like (ECL) cells in the gastric mucosa, representing approximately 1-2% of all gastric neoplasms. They are classified into three distinct types based on underlying pathophysiology. Type 1 (70-80% of cases) occurs in the setting of chronic atrophic gastritis and hypergastrinemia, is usually multifocal and small, and has excellent prognosis. Type 2 (5-10%) is associated with Zollinger-Ellison syndrome and MEN1, while Type 3 (15-20%) occurs sporadically, tends to be larger and more aggressive, and may metastasize. Treatment approaches vary significantly by type, from endoscopic surveillance for small Type 1 tumors to aggressive surgical resection for Type 3 lesions.
