Moderately differentiated is a histopathologic classification for intermediate-grade neuroendocrine tumors (Grade 2) characterized by preserved neuroendocrine architecture but increased proliferative activity compared to well-differentiated tumors. These tumors typically have mitotic counts of 2-20 per 10 HPF and Ki-67 proliferation indices of 3-20%. Moderately differentiated NETs represent an intermediate category between well-differentiated (Grade 1) and poorly differentiated (Grade 3) tumors in terms of both histologic features and clinical behavior. They generally have a better prognosis than poorly differentiated tumors but may require more aggressive treatment than well-differentiated NETs.
