Deliberate discontinuation of thyroid hormone replacement to stimulate TSH production for enhanced radioiodine therapy or diagnostic testing.

Full Definition

Thyroid hormone withdrawal involves the temporary cessation of levothyroxine therapy for 4-6 weeks to allow endogenous TSH levels to rise above 30 mIU/L, thereby stimulating iodine uptake by thyroid tissue and thyroid cancer cells. This induced hypothyroid state enhances the efficacy of radioactive iodine therapy and improves the sensitivity of thyroglobulin measurements. While effective, this approach causes significant hypothyroid symptoms and has largely been replaced by recombinant human TSH (rhTSH) stimulation in many clinical scenarios.
