Borderline resectable describes a category of pancreatic cancers that fall between clearly resectable and locally advanced unresectable disease. These tumors typically demonstrate limited involvement of major vessels such as the superior mesenteric vein, portal vein, or arterial structures that may be amenable to reconstruction. The National Comprehensive Cancer Network (NCCN) has established specific radiographic criteria for this classification. Patients with borderline resectable tumors are often candidates for neoadjuvant therapy to potentially downstage disease and improve R0 resection rates. This classification has become increasingly important in treatment planning and multidisciplinary decision-making.
