The Phalen test is a clinical examination technique used to diagnose carpal tunnel syndrome by reproducing median nerve compression symptoms. The test is performed by having the patient hold both wrists in complete flexion (either by pressing the dorsal surfaces of the hands together or allowing the wrists to fall into flexion under gravity) for 30-60 seconds. A positive test occurs when the patient experiences numbness, tingling, or pain in the median nerve distribution (thumb, index, middle, and radial half of ring finger). The maneuver increases pressure within the carpal tunnel, exacerbating median nerve compression in patients with carpal tunnel syndrome.
