The International Prognostic Index (IPI) is a widely validated prognostic model developed to predict outcomes in patients with aggressive non-Hodgkin lymphomas, particularly diffuse large B-cell lymphoma. The index incorporates five adverse prognostic factors: age >60 years, elevated lactate dehydrogenase, performance status ≥2, stage III-IV disease, and >1 extranodal site. Patients are stratified into low (0-1 factors), low-intermediate (2 factors), high-intermediate (3 factors), or high-risk (4-5 factors) categories, each associated with distinct survival expectations. The IPI guides treatment intensity decisions and serves as a stratification factor in clinical trials, though molecular markers are increasingly supplementing clinical prognostic models.
