Fractional flow reserve (FFR) is a lesion-specific index of coronary stenosis severity that measures the ratio of maximal achievable blood flow in a stenotic vessel to theoretical maximal flow in the same vessel if it were normal. The measurement involves advancing a pressure wire across the stenosis and administering adenosine to achieve maximal hyperemia, with an FFR value ≤0.80 generally considered hemodynamically significant and warranting revascularization. FFR assessment has revolutionized decision-making in coronary intervention by providing objective physiological data beyond angiographic appearance alone.
