Full Definition

Carmustine wafers (Gliadel wafers) are biodegradable polymer discs containing the alkylating agent carmustine (BCNU) that are implanted directly into the surgical cavity following brain tumor resection. These wafers slowly release chemotherapy over several weeks, delivering high local drug concentrations while minimizing systemic toxicity. The treatment is FDA-approved for high-grade gliomas and represents an example of local drug delivery in neuro-oncology. The biodegradable polymer gradually dissolves, eliminating the need for surgical removal. Common complications include cerebral edema, infection, and cerebrospinal fluid leak, which occur more frequently than with surgery alone.
