Neuraxis dissemination refers to the metastatic spread of primary brain tumors through cerebrospinal fluid pathways to distant sites within the central nervous system, including the brain and spinal cord. This pattern of spread is particularly common in certain pediatric brain tumors such as medulloepitheliomas, primitive neuroectodermal tumors, and some high-grade gliomas. The tumor cells travel through CSF circulation and can implant at various locations along the neuraxis, creating multiple lesions. Detection of neuraxis dissemination typically requires contrast-enhanced MRI of the entire brain and spine, and its presence significantly impacts staging, prognosis, and treatment planning, often necessitating craniospinal radiation therapy.
