Full Definition

Vincristine-induced peripheral neuropathy (VIPN) is a common and dose-limiting toxicity of vincristine, a vinca alkaloid frequently used in pediatric brain tumor protocols. The neuropathy results from microtubule disruption in peripheral nerves and typically presents as distal sensorimotor symptoms including numbness, tingling, and weakness beginning in the feet and hands. Symptoms usually develop after cumulative doses and may progress to significant functional impairment if treatment continues. The neuropathy can be partially reversible but may persist long-term, particularly in children who received high cumulative doses. Recognition and appropriate dose modification are essential to prevent severe and potentially permanent neurological dysfunction.
