Vertebrobasilar insufficiency (VBI) refers to inadequate blood flow through the vertebrobasilar arterial system, which supplies the brainstem, cerebellum, and posterior cerebral hemispheres. This condition can result from atherosclerotic stenosis, vertebral artery dissection, or subclavian steal syndrome. Clinical symptoms include dizziness, vertigo, diplopia, dysphagia, and drop attacks. Imaging findings may include chronic infarcts in the posterior circulation territory, vertebral artery stenosis on MRA or CTA, and flow voids in the basilar artery. Acute presentations may show restricted diffusion in the brainstem or cerebellum on DWI sequences.
