Myelin oligodendrocyte glycoprotein (MOG) is a glycoprotein found on the outermost surface of myelin sheaths in the central nervous system. It constitutes only a small percentage of total myelin protein but serves as an important target for autoimmune attack in certain neuroinflammatory conditions. MOG-associated disease (MOGAD) has emerged as a distinct clinical entity separate from multiple sclerosis and neuromyelitis optica, characterized by specific antibody responses against MOG. These antibodies can be detected through cell-based assays and are associated with particular clinical phenotypes including optic neuritis, acute disseminated encephalomyelitis, and brainstem encephalitis. MOG antibody-positive patients often show different treatment responses compared to other demyelinating diseases.
