Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disease affecting upper and lower motor neurons, leading to muscle weakness, atrophy, and eventual paralysis. In clinical documentation, ALS diagnosis requires evidence of both upper motor neuron signs (spasticity, hyperreflexia) and lower motor neuron signs (weakness, atrophy, fasciculations) in multiple body regions.
