Full Definition
Fetal engagement occurs when the presenting part, usually the fetal head, descends into the pelvic inlet and becomes fixed there. This typically happens in the final weeks of pregnancy in primigravidas or during early labour in multiparous women. Engagement is assessed by palpation and indicates that the fetal head has passed through the pelvic brim. In clinical documentation, editors should note that engagement is often described in terms of 'fifths palpable' above the symphysis pubis, with full engagement meaning no parts of the fetal head are palpable abdominally.
