Threatened abortion describes a clinical scenario in early pregnancy (before 20 weeks) where vaginal bleeding occurs but the cervix remains closed and fetal viability is maintained. Unlike inevitable abortion, the internal cervical os is closed on examination, and fetal cardiac activity may still be detected. The condition may progress to miscarriage or resolve completely. Management typically involves pelvic rest, serial beta-hCG monitoring, and ultrasound assessment. The bleeding is usually lighter than menstrual flow, and cramping may be absent or mild. Approximately 50% of threatened abortions will progress to complete miscarriage.
