Descemet's Stripping Automated Endothelial Keratoplasty (DSAEK) is a partial-thickness corneal transplant procedure that selectively replaces the diseased endothelium and Descemet's membrane while preserving the patient's healthy anterior corneal structures. This technique offers advantages over penetrating keratoplasty including faster visual recovery, stronger wound integrity, reduced astigmatism, and lower risk of rejection. The donor tissue is prepared using a microkeratome to create a posterior lamellar graft that is inserted through a small temporal incision and positioned using an air bubble for attachment.
