Cementoblastoma

A benign odontogenic tumour characterised by the formation of a mass of cementum-like tissue directly fused to the root of a tooth, almost exclusively in the mandibular molar region.
Full Definition
Cementoblastoma is the only true neoplasm of cementum recognised by the WHO. It presents as a radiopaque mass with a well-defined radiolucent rim intimately attached to a tooth root, most frequently the mandibular first molar. Resorption of the associated root is a hallmark feature. Histologically, the lesion shows sheets of cementum with reversal lines and a peripheral cellular zone of cementoblasts. Editors should note the fundamental distinction between cementoblastoma (neoplastic, fused to root) and hypercementosis (reactive, non-neoplastic deposition of cementum on the root surface).
Usage
Usage note: Distinguish carefully from 'hypercementosis,' which is reactive, not neoplastic. The diagnostic criterion of root fusion must be mentioned in reports; its omission should be queried.
