Significant gradients (typically >7-10%) indicate the presence of intracardiac shunts, with higher saturations downstream from left-to-right shunts. This measurement is crucial for quantifying shunt magnitude and determining the hemodynamic significance of congenital heart defects. The gradient analysis helps guide surgical decision-making and assess the need for intervention in patients with septal defects or other shunt lesions.
