The Childhood Health Assessment Questionnaire (CHAQ) is a widely used, validated instrument designed to assess functional ability and health-related quality of life in children with rheumatic diseases. The questionnaire consists of 30 items across eight domains including dressing, arising, eating, walking, hygiene, reach, grip, and activities. Parents or caregivers rate their child's ability to perform daily activities over the past week, providing a standardized measure of functional disability. The CHAQ has been adapted for various age groups and is considered an essential outcome measure in pediatric rheumatology clinical trials and routine clinical care.
