Caudal regression syndrome is a rare congenital disorder characterized by underdevelopment or absence of the lower spine, sacrum, and coccyx. The condition exists on a spectrum from mild sacral agenesis to complete absence of the sacrum and lower lumbar vertebrae. Associated anomalies commonly include neurogenic bladder and bowel dysfunction, lower extremity weakness or paralysis, and orthopedic deformities. The syndrome is strongly associated with maternal diabetes, occurring in approximately 1% of infants born to diabetic mothers versus 0.01% in the general population. Surgical management is multidisciplinary and may include spinal reconstruction, bladder augmentation, and orthopedic procedures.
