Transfer Assessment

Evaluation of a patient's ability to move safely between surfaces such as bed to chair or wheelchair to toilet.

Transfer assessment involves systematic evaluation of a patient's capacity to move independently or with assistance between different surfaces and positions. This includes bed mobility, sit-to-stand transitions, wheelchair transfers, and toilet transfers. Assessment considers safety, technique, level of assistance required, and use of adaptive equipment. Transfer skills are critical for discharge planning and determining appropriate living arrangements.
