A male fertility disorder characterized by the backward flow of semen into the bladder during orgasm, resulting in little to no antegrade ejaculation. This occurs when the internal sphincter of the bladder fails to close properly during ejaculation. Common causes include diabetes, certain medications (particularly alpha-blockers), spinal cord injuries, and previous surgeries affecting the bladder neck. Diagnosis is confirmed by finding sperm in post-ejaculatory urine samples. Treatment may involve medication to strengthen bladder neck closure, or sperm can be retrieved from alkalinized urine for use in assisted reproductive technologies.
