Sperm DNA fragmentation refers to breaks or damage in the DNA strands within sperm cells, which can significantly impact male fertility and reproductive outcomes. This condition is measured as the percentage of sperm with fragmented DNA in a semen sample, with levels above 20-30% (depending on the test used) considered elevated. DNA damage can result from various factors including oxidative stress, defective sperm packaging, abortive apoptosis, and exposure to toxins or heat. High levels of sperm DNA fragmentation are associated with reduced fertilization rates, poor embryo quality, increased miscarriage rates, and failed IVF cycles, even when conventional semen parameters appear normal. Testing methods include TUNEL assay, SCSA, and SCD test. Treatment may involve antioxidant therapy, lifestyle modifications, or surgical correction of underlying conditions like varicoceles.
