Heart failure with reduced ejection fraction (HFrEF) is a clinical syndrome characterized by impaired left ventricular systolic function (ejection fraction ≤40%) accompanied by signs and symptoms of heart failure. This classification has largely replaced the older term 'systolic heart failure' in contemporary cardiovascular research. HFrEF patients typically benefit from evidence-based therapies including ACE inhibitors, beta-blockers, and aldosterone antagonists. Editors should distinguish HFrEF from HFpEF (preserved ejection fraction) and HFmrEF (mildly reduced ejection fraction) in manuscript reviews.
