Dinutuximab is a chimeric monoclonal antibody that targets the GD2 ganglioside antigen highly expressed on neuroblastoma cells. It is administered as part of standard maintenance therapy for high-risk neuroblastoma patients following intensive chemotherapy and autologous stem cell transplant. Dinutuximab works through multiple mechanisms including complement-dependent cytotoxicity and antibody-dependent cellular cytotoxicity to eliminate residual neuroblastoma cells. The medication is associated with significant pain and requires premedication with opioids and antihistamines. Clinical trials have demonstrated improved event-free and overall survival when dinutuximab is combined with GM-CSF, IL-2, and isotretinoin in the maintenance setting.
