The Bosniak classification system is a widely used radiological framework for categorizing cystic renal lesions based on CT imaging characteristics and their associated risk of malignancy. The system includes categories I through IV, with subcategories IIF and III. Category I and II lesions are benign simple and minimally complex cysts requiring no follow-up. Category IIF lesions require imaging surveillance due to slightly increased complexity. Category III lesions have indeterminate malignant potential requiring surgical evaluation, while Category IV lesions are clearly malignant and require immediate treatment.
