Cabazitaxel is a semi-synthetic taxane chemotherapy drug approved for treatment of metastatic castration-resistant prostate cancer that has progressed after docetaxel-based chemotherapy. Unlike docetaxel, cabazitaxel has poor affinity for the P-glycoprotein efflux pump, potentially providing activity in docetaxel-resistant tumors. The drug is administered intravenously every three weeks and requires premedication to prevent hypersensitivity reactions. Common side effects include neutropenia, fatigue, neuropathy, and gastrointestinal toxicity. Cabazitaxel has demonstrated improved overall survival in the post-docetaxel setting.
