Castrate-resistant prostate cancer (CRPC) represents an advanced stage of prostate cancer that continues to grow and spread despite medical or surgical castration that reduces testosterone levels to less than 50 ng/dL (1.7 nmol/L). This condition develops through various mechanisms including intratumoral androgen synthesis, androgen receptor amplification, and alternative signaling pathways. CRPC can be further classified as non-metastatic (M0) or metastatic (M1) based on imaging findings. The development of CRPC typically occurs after an initial period of hormone sensitivity and represents a critical transition point requiring specialized treatment with novel androgen receptor inhibitors, chemotherapy, or immunotherapy. Median survival varies significantly based on extent of disease and treatment response.
