The cavernous nerves are bilateral autonomic nerve bundles containing parasympathetic fibers essential for erectile function, located along the posterolateral aspect of the prostate within the neurovascular bundles. These nerves originate from the pelvic plexus and travel to the corpora cavernosa of the penis, where they mediate smooth muscle relaxation and vasodilation necessary for penile erection. During radical prostatectomy, preservation of these nerves through nerve-sparing surgical techniques is crucial for maintaining postoperative erectile function. The anatomic relationship between the cavernous nerves and prostate cancer location influences the feasibility and extent of nerve-sparing approaches.
