Full Definition

Extracapsular extension (ECE) refers to the penetration of prostate cancer through the prostatic capsule into the periprostatic soft tissue. This finding indicates pT3a disease according to TNM staging and represents locally advanced prostate cancer. ECE can be detected preoperatively with multiparametric MRI or identified on final pathology after radical prostatectomy. The extent of extracapsular extension, whether focal or extensive, influences prognosis and treatment recommendations. Patients with ECE have higher risk of biochemical recurrence and may benefit from adjuvant radiation therapy.

Usage note: Often abbreviated as ECE. Distinguish from seminal vesicle invasion, which represents pT3b disease.
